About Our Assessment Services
Mental Health Evaluation
Mental health evaluations are a deep dive into one’s life in regards to mental health. With each evaluation, one can expect to receive consideration for a mental health diagnosis, assessments, and recommendations on what to do next. These evaluations include a clinical interview, where we ask you your life story, deep dive into mental health, and assessments such as those to measure personality, ability, as well as mental health.
Please note we specialize in open-ended mental health evaluations, meaning if you already know what you have and want confirmation, we are not the evaluators for you. We want to look at the whole person with limited biases, not just affirm what you already know.
We are not specialized in Autism Spectrum Disorder or Eating Disorders. If either diagnosis is the main concern, we may be able to provide referrals.
Intelligence Testing
We also provide stand alone intelligence testing such as Wechsler Adult Intelligence Scale (WAIS-IV) and Wechsler Intelligence Scale for Children (WISC-5).
We do not provide intelligence testing for reasons
outside of medical or educational uses.
Absolutely no diagnosis will be given for standalone
testing.

About Assessment Costs
Please Note: Life’s Hope Therapeutic Services accepts Medicaid, which will assist in covering costs for these services.
Mental Health Evaluation
A mental health evaluation starts at $900, which includes the clinical interview, standard assessments, and 4 hours of
administrative (report writing, collateral review, additional
interviews with individuals outside of client, and more).
Cognitive testing such as WAIS or WISQ add an additional $75 due to complexity.
Additional hours are $100/hour.
An estimate can be provided.
Intelligence Testing
Test proctor and report are $200 standalone.
Meet Your Evaluator
Kristen Weber, MA, LPCC
These evaluations and assessments are performed by Kristen Weber, a Licensed Professional Counselor Candidate, who has multiple years of experience in evaluation. Kristen is under supervision of Christina
Murphy, LPC and consults with a doctor-level consultant for evaluation and WAIS and WISC interpretation.
